International Applicants: Checklist - Freshman Applicant

In addition to your application, the information requested below must be submitted to complete the application process for your applicant level (freshman, transfer, or graduate).

Select forms are downloadable from the Admissions website, International menu under Requirements.

Transcripts


checkbox All official transcripts must be submitted in the native language and accompanied by a certified English translation with the translator certified by one of the following:

Following receipt of your transcripts, an independent evaluation may be required of your international credentials. Details may be viewed on your "To-Do" list on the Cal Poly Portal or navigate directly to "International Academic Evaluations"

Have your official transcripts/certificates and evidence of graduation from secondary school sent directly from the institution.

TOEFL (Test of English as a Foreign Language) or
IELTS (International English Language Testing System)

    • Taken within the last 2 years with a minimum score of 550 (paper version), 213 (computerized version), or 80 (internet based).

Financial Responsibility


checkbox Print this "Certification of Sponsor and Fee Schedule" form for your financial sponsor to sign.

Banking Statement


checkbox Submit a current banking statement in English verifying the amount of funds in U.S. dollars to support your educational expenses (must show minimum of $33,000.00 USD).

Health Insurance Proof


checkbox Print and submit the "Health Insurance Promissory Contract" certifying your promise to purchase an approved health insurance policy (see form for requirements).

Note: The health insurance policy you purchase is subject to approval by campus officials from the International Education Program before you register for classes should you be offered admission.

Education Background


checkbox Print form to relay your entire educational background from the beginning of your educational career to the present.

Spouse/Dependent Declaration


checkbox Print and submit this form to indicate whether you will or will not have a spouse and/or dependents who must travel with you to the United States during your course of study.
