Please Note: Former Graduate students who left Cal Poly before finishing their program, and wishing to resume the same program at Cal Poly, can essentially be reinstated after submitting an application through CSUMentor and appropriate documents if they have been gone for less than two years and their GPA was above 3.00.

However, if a former Graduate student has not been enrolled for more than two years, has less than a 3.00 gpa or both, their CSUMentor application and supporting documents must be sent to that program's graduate coordinator for review before reinstatement can occur.
